Description:
The Community Outreach Specialist is responsible for building relationships with key contacts at assisted living facilities, nursing home facilities, hospitals, and physician offices for adding clients to State of the Heart Care programs. The Community Outreach Specialist is responsible for educating the community and referral sources about State of the Heart services. Responsibilities:• Develop profiles for referral sources to include; current key personnel, current occupancy, needs assessment, and track referral statistics• Prospect and market to targeted referral sources through various outreach activities• Coordinate training needs for referral sources with appropriate presenters• Participate in community programs, events, and health fairs• Serves as the primary marketing resource• In conjunction with the Director of Business Development prepare a goal-directed development plan and quarterly marketing plan• Work collaboratively with the Business Development Team and the Admissions Specialist to identify and address issues with referral sources• Utilize and analyze the referral source database• Other duties as assigned Qualifications:• High school diploma or associate degree in business or a related field• Minimum of two years of Hospice or community health experience• Healthcare sales and/or marketing experience preferred About Company: EverHeart Hospice was established in 1980 and is a legacy non-profit, community-based hospice organization.
